Pandemic
2020년 03월 11일 WHO (World Health Organization) 스위스 제네바에서 코로나19를 전염병최고 등급인 Phase 6 으로 격상하여 분류하겠다고 공식적으로 선언했습니다.
역사상 WHO가 공식적으로 선언한 Phase 6 인 Pandemic은, 1968년의 홍콩독감과 약 10년전 ,2009년 유행했던 신종플루에 이어서 총 3번 째의 판데믹 선언입니다. 홍콩독감으로는 현재까지 100만명이 사망하였고, 신종플루는 약 2만명이 사망했습니다.
코로나19 바이러스에 대한 WHO의 발표는 아래와 같이 진행 되었습니다.
- 2020년 01월 30일 발표한 '국제적 공중 보건 비상 사태' (Public Health Emergency of International Concern, PHEIC), PHEIC는 이번이 6번 째선포입니다.
- 2020년 02월 28일 글로벌 위험도 '매우높음'으로 상향
- 2020년 03월 11일 Pandemic 선언
전염병의 단계정립
WHO의 전염병 단계는 1999년에 정립되어 2005에 개정되었습니다. '단계'들은 전세계에 적용을 할 수 있으며, 또한 국가들의 전염병에 대한 대응 대첵과 대비를 수립할 수 있도록 국제적인 프레임워크(틀)을 제공하게 합니다. 2005년 개정판에서는 이러한 대응 계획과 접근방식 그리고 새롭게 권장되는 사항에 대해 통합하여 6가지 단계로 정립을 하였습니다.
1~3단계에서는 전염병에 대비하기 위한 능력개발과 대응계획 활동과 같은 준비와 관련이 있으며, 4~6단계에서는 전염병에 대한 대응이나 완화 혹은 피해를 최소화 하기위한 노력을 하게 되있습니다. 결론은 세계보건기구는 해당 전염병의 각각의 위험도에 따라서 이런 위험 경보 등급을 1단계 ~ 6단계로 나누었습니다. 뒤로 POST PEAK 과 POST PANDEMIC의 단계도 있는데, 밑에서 하나씩 살펴 보도록 하겠습니다. 😃
1단계 (Phase 1)
*동물간에 전염되는 바이러스가 인간에게 전염되는 사례가 없습니다.
Phase 1 no viruses circulating among animals have been reported to cause infections in humans.
2단계 (Phase 2)
*가축 등이나 야생의 동물들에게서 발견 되며, 소수의 사람들에게 옮길 수도 있는 가능성이 있는 단계. 가축 또는 야생 동물 사이에서 순환하는 동물 인플루엔자 바이러스는 인간에게 감염을 일으키는 것으로 알려져 있으므로 잠재적 인 유행성 전염병으로 간주됩니다.
Phase 2, an animal influenza virus circulating among domesticated or wild animals is known to have caused infection in humans and is therefore considered a potential pandemic threat.
3단계 (Phase 3)
사람/인간에게 전염이 되었지만 소규모 집단을 감염 시켰거나 사람과 사람사이의 감염을 일으키지 않는 전염병의 단계입니다.
Phase 3, an animal or human-animal influenza reassortant virus has caused sporadic cases or small clusters of disease in people but has not resulted in human-to-human transmission sufficient to sustain community-level outbreaks.
Limited human-to-human transmission may occur under some circumstances, for example, when there is close contact between an infected person and an unprotected caregiver. However, limited transmission under such restricted circumstances does not indicate that the virus has gained the level of transmissibility among humans necessary to cause a pandemic.
4단계 (Phase 4)
사람과 사람 사이에서 전염병이 옮기기 시작하고 번지며, 유행이 될 가능성이 현저하게 커진 단계를 말합니다. 이 단계의 전염병이 확산된 국가는 WHO와 협의가 필요하며, 이러한 전염병을 막기위한 국가적인 구체적 조치를 강구해야하고, 구체적인 조치로는 여행자제 등이 있습니다.
Phase 4 is characterized by the verified human-to-human transmission of an animal or human-animal influenza reassortant virus able to cause “community-level outbreaks”. The ability to cause sustained disease outbreaks in a community marks a significant upwards shift in the risk of a pandemic.
Any country that suspects or has verified such an event should urgently consult with WHO so that the situation can be jointly assessed and a decision made by the affected country if the implementation of a rapid pandemic containment operation is warranted. Phase 4 indicates a significant increase in the risk of a pandemic but does not necessarily mean that a pandemic is a forgone conclusion.
5단계 (Phase 5)
사람과 사람간의 바이러스 퍼지는 전염병이 동일 대륙(WHO지역이 기준(사진참조))에서 두 개 나라(국가) 이상에서 발생한 상태이며 Epidemic 즉 감염병이 유행한다고 하는 단계입니다. 반면 대다수의 국가들은 아직 감염의 영향이 없는 단계이며, 5단계란 판데믹이 될 수 있는 강력한 신호로서 많은 준비를 해야할 시기입니다.
Phase 5 is characterized by human-to-human spread of the virus into at least two countries in one WHO region (Figure). While most countries will not be affected at this stage, the declaration of Phase 5 is a strong signal that a pandemic is imminent and that the time to finalize the organization, communication, and implementation of the planned mitigation measures is short.
6단계 (Phase 6)
마지막 단계는 Pandemic 이며, 감염병이 세계적으로 유행했을 때의 단계입니다. 판데믹 단계인 6 단계는 5 단계에 정의 된 기준 외에 다른 WHO 지역의 다른 국가에서 적어도 하나의 다른 국가에서 발생하는 지역 사회 수준의 확산이 특징입니다. 이 단계로의 전염병경도 격상은 전 세계적으로 유행하는 전염병이 진행 중임을 나타냅니다.
Phase 6, the pandemic phase, is characterized by community-level outbreaks in at least one other country in a different WHO region in addition to the criteria defined in Phase 5. Designation of this phase will indicate that a global pandemic is underway.
POST PEAK 과 POST PANDEMIC
전염병이 정점을 찍고 난 뒤에 단계도 있습니다.
- Post Peak: Possibility of Recurrent Events. 재발할 가능성이 있는 단계이며,
- Post Pandemic: Disease Activity at Seasonal Levels. 계절성있는 레벨에서의 질병이 활동하는 단계를 말합니다.
